# Therapy for Doctors

For physicians and surgeons, residents and attendings, who carry the decision and the outcome long after the chart is closed.

## The Weight of Being the One Who Decides

You were trained to hold responsibility, and you do. The differential, the timing, the call to operate or wait, the conversation with the family. Most of the time it goes as well as medicine allows. What stays is the small number of times it did not, and the even smaller number where you are still not certain whether a different decision would have changed anything.

That uncertainty is the part that does not resolve on its own. It is not the same as the fatigue of a long stretch of shifts. It is a specific case, a specific room, a specific face, and it comes back at three in the morning or in the middle of an unrelated clinic day.

You know the medicine. Knowing it has not made the memory quieter.

## Why Physicians Wait

The most common reason I hear is not stigma in the abstract. It is a practical worry that seeking treatment becomes a licensing or credentialing problem: something to disclose, something that follows the application, something that turns a private difficulty into an administrative one.

I am not in a position to advise you on how any board or credentialing body handles that, and I will not pretend otherwise. What I can tell you is what this practice is. It is private, it is by secure video, it is not affiliated with any hospital, system or program, and it produces no report to anyone. I hold your confidentiality within the ordinary legal limits I go over before we begin, and nothing leaves without your written authorization.

The second reason is time, in a schedule that was never built with room in it. That is part of why the intensive format exists.

## What a Session Looks Like Here

We choose a target, usually a specific case or moment rather than a general sense of strain, and I guide the eye movements while you hold what comes up. You control how much you say out loud. You do not have to narrate the case in detail, which matters to people who have already presented it at morbidity and mortality and do not want to tell it again.

Sessions are 60 minutes by secure video, or up to three hours as [an ART intensive](/art-intensives), which suits a schedule where one protected block is easier to find than a weekly hour. If what you are carrying is less about a particular decision and more about cumulative exposure across shifts, the [page for EMTs, ER and healthcare workers](/who-i-work-with/healthcare-workers) is probably the better fit, and either page reaches the same practice.

## What This Is Not, and When It Is Not the Fit

-   Not a fitness for duty evaluation, a return to work clearance, or an assessment for a board, program or credentialing body. 
-   Not legal advice about disclosure, licensure or credentialing, and not a substitute for your own counsel.
