# Therapy for EMTs, ER and Healthcare Workers

For the people who witness it as part of the job, repeatedly, and then walk into the next room and start again.

## Exposure That Never Looked Like One Event

This rarely presents as a single trauma, which is exactly why it goes untreated. It is the accumulation: the code that did not go your way, the family in the corridor, the trauma bay at 3 a.m., the patient who was the same age as your daughter. Then a bed request, a chart, and the next room.

For EMTs and medics it is the scenes with no doors between them and the rest of the day. For nurses it is the hours of proximity, being the one in the room when it happened and again when the family arrived. For attendings and residents it is the decisions you are still auditing months later, at 4 a.m., in bed.

Signs are unglamorous. You are numb where you used to feel something. Charting takes longer. You are irritable at home and flat at work. You cannot watch medical scenes on television anymore. Somewhere in there is a specific case you have never told anyone about, that arrives on its own.

## Why Clinical Staff Put This Off

You know what to do about this. You have given a version of the advice to patients. Knowing the recommendation and being on the receiving end of it are different tasks, and the second one requires admitting the exposure landed.

There are practical barriers too, and they are real: rotating shifts that make a standing weekly appointment impossible, and a strong preference not to be treated inside the system you work in. Secure video handles the second. For the first, I hold sessions outside standard hours where I can, and [an ART intensive](/art-intensives) lets you use one stretch of time instead of finding twelve of them.

## What a Session Looks Like Here

We start with the cases that come back uninvited, and usually there are two or three doing most of the work even after years of shifts. We take them one at a time. You hold the image while I guide the eye movements.

You are not required to explain the medicine to me or justify the call you made. I am not reviewing the case, and nothing here is a morbidity and mortality conference. Sessions are 60 minutes by secure video, and up to three hours as an ART intensive. This is private pay and it lives entirely outside your employer, your credentialing and your hospital record.

## What This Is Not, and When It Is Not the Fit

-   Not case review, peer review, or anything connected to a quality or risk process. 
-   Not an employee assistance program, and I am not contracted with any hospital or health system. 
-   Not documentation for licensure, credentialing or a health program, and I do not write those letters. 
-   Not a claim about any outcome, and never a promised number of sessions. 
-   Adults only, by secure video in Kentucky and Ohio. 

This is not a crisis service.
